Kratom Research 2025/2026: Where Do We Stand?
What We Know with High Confidence
- Mitragynine is the main alkaloid, typically up to 66% of the alkaloid content
- 7-hydroxymitragynine occurs in small amounts and shows higher μ-MOR affinity than mitragynine
- Both alkaloids are partial agonists at the μ-opioid receptor in preclinical models
- Mitragynine is metabolized in the liver via CYP enzymes
- Kratom has dependence potential – withdrawal symptoms after regular use are documented
What We Know with Moderate Confidence
- Kratom reduces classical opioid withdrawal symptoms in animal models
- Self-report surveys: a considerable share of users report using kratom for opioid cessation
- Alkaloid profiles vary considerably by origin, harvest and processing
- Heavy metal and pesticide contamination is a documented quality problem
What We Do Not Know (Research Gaps)
- Safe limits in humans – no clinically validated thresholds
- Long-term consequences of regular kratom use
- Whether kratom is clinically effective and safe as substitution therapy (RCTs missing)
- The complete interaction profile with other medications
- Whether and to what extent kratom is hepatotoxic
Ongoing and Planned Research
Johns Hopkins University has been carrying out kratom research for several years – surveys, case reports, preclinical work. The difficulty: kratom is not scheduled, but neither is it officially recognized as a research substance. That makes funding and controlled clinical trials harder to obtain.
Conclusion
Kratom research has developed, but remains far behind what would be needed for well-founded health policy decisions. The research deficit is in part the result of political blockades. Kratom deserves more serious research, not less.
