Two Risk Dimensions
1. Serotonin Syndrome: Mitragynine shows preclinical hints of serotonin receptor interactions (5-HT2A, 5-HT7). With simultaneous SSRI/SNRI intake, serotonerging activity could summate – risk not definitively established but not excluded. Published case reports describe a possible association.
2. Cytochrome P450 Interaction: Mitragynine may inhibit CYP3A4 and CYP2D6 – the same enzymes metabolising many antidepressants. This could lead to elevated antidepressant blood levels with unpredictable effects.
