Kratom & Benzodiazepines: What Does Research Say?

If there is one substance combination discussed with particular seriousness in the kratom community, it is this one: kratom and benzodiazepines. Unlike some other combinations, there are concrete and consistent data here that urge caution.

What Are Benzodiazepines?

Benzodiazepines (diazepam/Valium, alprazolam/Xanax, lorazepam/Ativan, clonazepam/Klonopin) bind to GABAₐ receptors and enhance the inhibitory effect of GABA – the main inhibitory neurotransmitter in the CNS – producing comprehensive CNS depression.

Why the Interaction Is Critical

The critical point: both substance classes can cause respiratory depression. Benzodiazepines via the GABA pathway, opioids (and opioid-like substances such as mitragynine) via the opioid pathway. When both mechanisms are simultaneously activated, the risk of life-threatening respiratory depression is considerably higher than with either substance alone.

What the Data Show

FAERS database analyses show benzodiazepines were among the most frequently co-involved substances in serious adverse event reports and fatalities associated with kratom. Published case reports describe respiratory arrest or severe sedation following this combination. Naloxone was partially effective in some cases.

Conclusion

Kratom and benzodiazepines are the combination against which research data most consistently warn. Two substance classes that can produce CNS depression and respiratory depression through different mechanisms should not be combined.
